Nurturing Care Partners was founded by Titilayo (Titi) Ayanwola, MPH, RD, LD, a credentialed Registered and Licensed Dietitian with a Master of Public Health in Human Nutrition from the University of Michigan. The ritual of a thoughtfully prepared meal, eaten at the right time, with the right foods, in good company, does something that no supplement or clinical intervention fully replicates. Our nutrition and meal support service goes well beyond cooking. Our caregivers are trained to notice when appetite changes, to prepare meals that respect individual dietary needs and health goals, and to create the kind of mealtime experience that genuinely nourishes, not just sustains.

Post-Hospital Recovery
Adequate nutrition after hospitalization is essential to recovery, healing, and reducing the risk of readmission. We prepare nourishing, recovery-supportive meals and monitor appetite and intake changes during the critical weeks following discharge.

Diabetes-Related Dietary Needs
For clients managing diabetes, we prepare meals mindful of carbohydrate balance, portion consistency, and meal timing, following the guidance of the client's physician or registered dietitian. We do not provide clinical nutrition therapy or diabetes management.

Heart Health and Cardiac Diets
We support clients following physician-recommended cardiac diets by preparing meals that are lower in sodium, saturated fat, and processed ingredients, while remaining delicious, satisfying, and aligned with the individual's personal preferences.

Dementia and Appetite Decline
Appetite loss and eating difficulties are common in dementia. Our caregivers are trained to recognize early warning signs, offer meals in gentle and encouraging ways, support preferred foods and familiar routines, and report changes to the family and care team.

Weight and Malnutrition Prevention
Unintended weight loss and malnutrition are serious risks for older adults living alone. We prioritize consistent, nourishing meals and snacks, actively monitor intake, and alert families and coordinators when concerning patterns emerge.

Swallowing and Texture Needs
For clients with dysphagia or chewing difficulties, we prepare texture-modified meals following the recommendations of the client's physician, speech-language pathologist, or other treating clinician. Safety and dignity are both priorities at every meal.
An important distinction: Nurturing Care Partners provides non-medical home care. Our nutrition and meal support services are not a substitute for clinical nutrition therapy, medical dietary assessment, or treatment prescribed by a licensed healthcare provider. All dietary support is provided in coordination with your healthcare team. Our founder's credentials as an RD, LD inform the standards and training of our agency, and do not constitute a clinical provider relationship.

about nutrition support

Is this the same as seeing a dietitian?
No. Our nutrition and meal support is a non-medical home care service. It is not clinical nutrition therapy and does not replace the guidance of your loved one's physician or registered dietitian. What it provides is knowledgeable, attentive meal support informed by our founder's RD, LD credentials and built into the training of every caregiver we place.
Can your caregivers follow a specific diet prescribed by a doctor?
Yes, within the scope of non-medical home care. Our caregivers can prepare meals that follow physician-recommended dietary guidelines, such as low-sodium, low-sugar, or texture-modified diets. They do not provide clinical dietary counseling or medical nutrition therapy.
What if my loved one has a very specific cultural or religious diet?
Cultural and religious dietary preferences are built into the care plan from the very beginning. We take these seriously and match caregivers who are comfortable preparing the relevant foods wherever possible. Honoring individual heritage at mealtimes is part of how we respect the whole person.
My parent has lost a lot of weight. Can you help?
Unintended weight loss in older adults is a serious concern we take very seriously. Our caregivers are trained to prioritize consistent, nourishing meals and to monitor intake carefully. We also communicate intake changes to family members and care coordinators promptly. If clinical evaluation is needed, we encourage and support that conversation with the appropriate healthcare provider.
Do you do grocery shopping?
Yes. Grocery coordination and shopping are part of our nutrition support service. Caregivers shop based on the care plan, individual preferences, and any dietary guidelines provided by the healthcare team. Pantry organization and food safety monitoring are also included.
